COVID-19 Policies/Procedures, Prescreening, and Liability Waiver
We are taking precautions and actions to be as healthy and happy as possible.

COVID-19 Policies/Procedures
Watching for Symptoms
According to the CDC, people with COVID-19 have had a wide range of symptoms reported – ranging from mild symptoms to severe illness. Symptoms may appear 2-14 days after exposure to the virus. People with these symptoms may have COVID-19:
Cough
Shortness of breath or difficulty breathing
Fever or chills
Fatigue
Muscle pain or body aches
Headache
Sore throat
New loss of taste or smell
Congestion or runny nose
This list is not all possible symptoms. Other less common symptoms have been reported, including gastrointestinal symptoms like nausea, vomiting, or diarrhea.
If you or anyone in your household is experiencing COVID-19 symptoms, or have been in contact with anyone with a positive test, or are waiting on results from a test, please get in touch so we can reschedule your session after a 14-day period has passed.
Travel
If you or anyone in your household has traveled by air recently, please get in touch so we can reschedule your session after a 14-day period has passed.
Social Distancing Measures
Following the CDC's guidance, Quincy Street Kitchen (Delicious LLC) requires that you comply with the following protocols and procedures while in the studio or on location:
Maintain a six-foot distance at all times (not including household members or situations where closeness is allowed)
No physical greetings (Waving or air high-fives are greatly appreciated!)
Health and Safety
Masks are required at all times indoors. This includes: entering and exiting the building and when you’re not being photographed.
The most effective types of face coverings can include a paper or disposable mask or a cloth mask.
The CDC recommends masks with two or more layers of tightly-woven fabric. Face coverings made of thinner, loosely woven, or single-layer fabric such as certain types of masks, scarves, neck gaiters, bandannas, or face shields are not as effective for blocking droplets that come out when speaking, coughing, or sneezing, and should only be used if nothing else is available. Photography team will provide disposable masks for anyone who does not have one.
Photography team will wear a mask or face-covering in the studio.
Photography team will provide hand sanitizer to everyone upon entering the studio.
Photographer will use hand sanitizer or thoroughly wash hands before and after each session.
High touch surfaces (i.e door handles) will be cleaned and disinfected on a regular schedule and between sessions.
Photography team will move equipment, gear, and props.
Long zoom lenses will be used to allow for social distancing and directions/adjustments will be verbal (not hands-on)
With a goal of “maximum work in minimum time,” sessions may be shortened to minimize the time in the studio or on-site
Proof of Vaccination or Negative Test Results
For all in-studio productions, we are requiring either proof of vaccination or a negative test result (the test has to be taken a maximum of 48 hrs before the start of the shoot) for each person on set (clients and crew). This is especially important when one or more individuals will be unmasked while being featured in a photo or video. Please forward your test results to Kristen@QuincyStreetKitchen.com. Anyone who has not received and provided their results before the start of the photoshoot will not be allowed on set.

Daily Screening Questionnaire
THIS SCREENING MUST BE FILLED OUT WITHIN 24 HOURS OF YOUR SESSION START TIME.
The screening protocol outlined below is based on the recommendations by the CDC.
COVID-19 Liability Release Waiver for Clients:
Due to the 2020-2021 outbreak of the novel Coronavirus (COVID-19), Quincy Street Kitchen (Delicious LLC) is taking extra precautions with the care of every client to include health history review and enhanced sanitation/disinfecting procedures in compliance with CDC guidance.
I, the Client, agree to the following:
• I understand the symptoms listed above and affirm that I, as well as all household members, do not currently have, nor have experienced the symptoms listed above within the last 14 days.
• I affirm that I, as well as all household members, have not been diagnosed with COVID-19 within the past 30 days.
• I affirm that I, as well as all household members, have not knowingly been exposed to anyone diagnosed with COVID-19 within the past 30 days.
• I affirm that I, as well as all household members, have not traveled outside of the country or to any location considered to be a “hot spot” for COVID-19 infections within the past 30-days.
• I understand that Quincy Street Kitchen (Delicious LLC) cannot be held liable for any exposure to the COVID-19 virus caused by misinformation on this form or the health history provided by each client.
